skills. Use the scale below to indicate how often each statement
applies to you. Be as honest as you can in responding since that
will provide you with the most useful information. Place the number
corresponding to your choice in the blank provided. At the end of
each section, total the column. Pay attention to your highest and
lowest scores as they will indicate stronger and weaker areas. Note,
this questionnaire is not absolute. It should be used only to
give you a general idea of how you view your abilities.
